Some features are unattractive in either sex. Prominent frontal bossing is one such problem, even though men enjoy a wider range of acceptablilty than women do. When the bony overgrowth is excessive, it can be filed down during a brow lift. In unusual circumstances when the contour is due to enlargement of the frontal sinus, more extensive surgery may be necessary.

This is a good example of excessive frontal bossing corrected by Dr. Brink. The browlift procedure was done with an incision behind the hairline. Excess bone was contoured and the hairline was raised.

This patient demonstrates that some people, men or women, look better with higher hairlines. Even though hairlines are generally lower in females than in males, lowering the hairline is usually optional for transgender patients.

The ideal position of the anterior hair line is a matter to be taken into consideration when planning a brow lift because it can be moved forward, backward or left unchanged. Although lower hairlines are more typically feminine, this does not necessarily mean that lower is better for every patient.

Some female patients look better with higher hairlines.

This genetic female's main complaint was a hairline that was too low. It was raised at the time of browlift. This patient and the previous one make the point that even though female hairlines are generally lower than male hairlines, some people look better with a "taller forehead".

Transgender patients should be aware of the options available to them.
